چکیده:
اهداف: هدف از این مطالعه، بررسی فراوانی و نحوه توزیع جنسی و سنی عوامل خطر قابل اصلاح مرتبط با سندرم حاد کرونری در بیماران بستری در یکی از بیمارستانهای منتخب نیروی انتظامی بود.
مواد و روشها: این مطالعه توصیفی- مقطعی در کلیه بیماران بستریشده در بخش سیسییو یکی از بیمارستانهای منتخب نیروی انتظامی با تشخیص سندرم حاد کرونری بین سالهای ۱۳۹۷-۱۳۹۶ انجام شد. دادههای اولیه و زمینهای بیماران شامل جنس، سن و عوامل خطر مرتبط با سندرم حاد کرونری (شامل فشار خون بالا، غلظت چربی بالا، دیابت، مصرف سیگار، مصرف اپیوم و چاقی) گردآوری شدند. مقایسه متغیرهای کمی با استفاده از t-test و مقایسه متغیرهای کیفی با استفاده از chi-square test در قالب نرمافزار SPSS ۲۳ انجام شد. در پایان، مقایسه فراوانی فاکتورهای خطر قابل اصلاح با جامعه آماری مشابه در ایران، ترکیه و همچنین با جامعه فاقد بیماری قلبی ایرانی با استفاده از آزمون مقایسه نسبتها بررسی شد.
یافتهها: در مجموع ۶۱۲ بیمار مبتلا به سندرم حاد کرونری با میانگین سنی ۸۸/۱۱±۶۰/۶۲ سال (در محدوده ۲۷ تا ۹۳ سال) وارد مطالعه شدند که ۳۴۹ مورد (۰/۵۷%) مرد بودند. فشار خون بالا (۲/۶۶%)، چربی خون بالا (۱/۴۳%) و دیابت ملیتوس (۰/۴۲%) به ترتیب شایعترین ریسک فاکتورها در مطالعه حاضر بودند. همبستگی معنادار اما معکوسی بین سن بیماران و شاخص توده بدنی آنان برقرار بود (۱۷۹/۰-=r؛ ۰۰۱/۰> p). شیوع فشارخون بالا، چربی خون بالا، دیابت ملیتوس و چاقی نسبت به جمعیت مشابه در ترکیه و جمعیت فاقد سندرم حاد کرونری در ایران تفاوت معنادار داشت. همچنین شیوع فشار خون بالا و چاقی نسبت به جامعه دارای سندرم حاد کرونری در ایران افزایش معنادار داشت ولی چربی خون بالا و دیابت تفاوت معنادار نداشت.
نتیجهگیری: شایعترین فاکتورهای قلبی و عروقی اصلاحپذیر در بین جامعه منتخب مطالعه حاضر شامل فشار خون بالا، چربی خون بالا، دیابت ملیتوس و چاقی است. فراوانی هر چهار ریسک فاکتور شایع در زنان بیشتر از مردان است. مصرف سیگار و چاقی در بیماران جوانتر و عامل فشار خون بالا در بیماران مسنتر شایعتر است.
AIMS. The aim of this study was to evaluate the frequency and the sexual and age distribution of modifiable risk factors associated with acute coronary syndrome in patients admitted to one of the selected hospitals of police force.
MATERIALS & METHODS. This descriptive cross-sectional study was performed on all patients admitted to CCU of one of the selected hospitals of police force with the diagnosis of acute coronary syndrome during 2017-18. Baseline data on patients including sex, age, risk factors for acute coronary syndrome (including hypertension, hyperlipidemia, diabetes, smoking, opium use, and obesity) were collected. T-test and chi-squared test were respectively used for quantitative and qualitative variables comparison using SPSS 23. Finally, comparing the frequency of modifiable risk factors among the similar population in Iran and Turkey as well as population without acute coronary syndrome was done.
FINDINGS. A total of 612 patients with acute coronary syndrome with mean age of 62.60±11.88 years (range 27 to 93 years) were enrolled that 349 persons (57.0%) were male. Hypertension (66.2%), hyperlipidemia (43.1%) and diabetes mellitus (42.0%) were the most common risk factors in the present study, respectively. There was a significant but inverse correlation between age and BMI (r=-0.179; p<0.001). The frequency of hypertension, hyperlipidemia, diabetes mellitus, and obesity was significantly different from similar populations in Turkey and those without acute coronary syndrome in Iran. The frequency of hypertension and obesity was also significantly increased in the population with acute coronary syndrome in Iran, but hyperlipidemia and diabetes were not significantly different.
CONCLUSION. The most common modifiable cardiovascular factors among the selected population were hypertension, hyperlipidemia, diabetes mellitus and obesity. All four risk factors are more common in women than in men. Cigarette smoking and obesity are more common in younger patients and hypertensive in older patients.
