چکیده:
زمینه: ادراک بیماری، بازنمایی های سازمانیافته شناختی می باشد که بیماران تالاسمی از بیماری شان و اثرات آن دارند. پژوهش ها نشان میدهد برنامه کاهش استرس مبتنی بر ذهن آگاهی بر خودمدیریتی و پریشانی بیماران مؤثر است اما کاربرد این درمان بر ادراک بیماری در جامعه مورد نظر شکاف تحقیقاتی می باشد. هدف: پژوهش حاضر با هدف بررسی اثربخشی برنامه کاهش استرس مبتنی بر ذهن آگاهی بر ادراک بیماری در بیماران تالاسمی انجام شد. روش: روش پژوهش شبه آزمایشی با طرح پیش آزمون – پس آزمون با گروه کنترل بود. جامعه آماری پژوهش را کلیۀ بیماران مبتلا به تالاسمی شهر بهشهر که در سال 1400 عضو انجمن تالاسمی بودند، تشکیل دادند (تقریباً 90 نفر). تعداد 40 نفر به روش نمونه گیری هدفمند بهعنوان نمونه پژوهش انتخاب و بهتصادف در دو گروه آزمایش و کنترل جایگزین شدند (هر گروه 20 نفر). ابتدا هر دو گروه به پرسشنامه ادراک بیماری برادبنت و همکاران (2006) پاسخ دادند (پیش آزمون). سپس گروه آزمایش به مدت 8 جلسه تحت برنامه کاهش استرس مبتنی بر ذهن آگاهی کابات زین (2009) قرار گرفتند و گروه کنترل مداخله ای دریافت نکرد. تجزیه و تحلیل اطلاعات با استفاده از روش آماری تحلیل کوواریانس چندمتغیره انجام شد. یافته ها: نتایج نشان داد در مرحله پس آزمون نمره مؤلفههای ادراک بیماری در پیامدها، طول مدت، ماهیت، نگرانی، پاسخهای عاطفی و علت بیماری، افراد گروه آزمایش نسبت به گروه کنترل کاهش معنی داری یافته است (0/05 >p) و در مؤلفههای کنترل شخصی، کنترل درمان و شناخت بیماری افزایشیافته است که ناشی از اثربخشی برنامه کاهش استرس مبتنی بر ذهن آگاهی بر ادراک بیماری بود. نتیجهگیری: برنامه کاهش استرس مبتنی بر ذهن آگاهی در بیماران تالاسمی سبب کاهش نگرانی و تلاش بیشتر آنان در جهت کنترل شرایط بیماریشان گردید و لذا آگاهی و استفاده از این رویکرد جهت ادراک از بیماری در افراد مبتلا به تالاسمی میتواند کمککننده باشد.
Background: Disease perception is an organized cognitive representation that thalassemia patients have of their illness and its effects. Research has shown that mindfulness-based stress reduction programs are effective on self-management and reducing patient anxiety, but the application of this treatment on disease perception in the target population is a research gap.
Aims: The aim of this study was to evaluate the efficacy of mindfulness-based stress reduction program (MBSR) on disease perception in thalassemia patients.
Methods: The research method is quasi-experimental with pre-test-post-test design with control group. The statistical population of the study consisted of all patients with thalassemia in Behshahr who were members of the Thalassemia Association in 1400 (approximately 90 people). Fourty thalassemia patients selected by purposive sampling as a research sample and randomly assigned to experimental and control groups (20 patients in each group). First, both groups answered the Bradbent et al.'s (2006) Perception of Disease Questionnaire (pre-test). The experimental group then underwent mindfulness-based stress reduction program of Kabat Zinn (2009) for 8 sessions and the control group did not receive any intervention. Data analysis was performed using multivariate analysis of covariance.
Results: The results showed that in the post-test stage, the score of disease perception components in the outcomes, duration, nature, concern, emotional responses and cause of the disease, the subjects in the experimental group compared to the control group decreased significantly (P
Conclusion: Mindfulness-based stress reduction program (MBSR) for thalassemia patients reduces their anxiety, and leads to their greater efforts to control their disease status.
